Good afternoon,

The federal government now requires health insurance plans to cover the cost of over-the-counter COVID-19 tests for all covered individuals beginning January 15, 2022. This policy will remain in effect for the duration of the Federal Public Health Emergency. 

What does this mean for you?

If you are currently enrolled in 新沙巴体育投注_足球比分直播-平台*网址’s health insurance plan, CareFirst BlueCross BlueShield will now reimburse you for the cost of up to eight over-the-counter at-home COVID-19 tests for each covered individual every 30 days.

  • You do not need a healthcare provider order or clinical assessment in order to be reimbursed.

  • If multiple tests are sold in one package, i.e., if one package includes two tests, it counts as two tests toward the quantity limit.

  • Tests that can be purchased at a retail location or online but are then sent to a lab for processing are not covered unless accompanied by a healthcare provider order.

To be eligible for reimbursement, the COVID-19 tests must:

  • Be purchased on or after January 15, 2022.

  • Be for personal diagnostic use to identify the potential COVID-19 infection.

  • Not be for employment purposes or surveillance testing.

  • Be self-administered with results that can be self-read.

  • Not be resold, given, or supplied to individuals not covered under your health insurance policy.

  • Not be reimbursed by another source.

Process to submit your reimbursement claim:

All claims for reimbursement must be submitted directly to CareFirst BlueCross BlueShield.

  • Step 1: Complete and sign the attached claim form including the attestation.

  • Step 2: Enclose the required documentation:

    • The purchase receipt documenting the date of purchase and the price.

    • The Universal Product Code from the over-the-counter COVID-19 test packaging (The UPC is the barcode and 12-digit-number that typically runs below it.)

    • The signed and completed attestation form confirming and acknowledging that the purchased over-the-counter at-home COVID-19 test meets eligibility guidelines.

  • Step 3: The Claim form and all related documents should be mailed to:

    • CareFirst BlueChoice, Inc.

Other Testing Options

CareFirst will continue to cover the cost of COVID-19 tests administered by a healthcare provider for diagnostic purposes and you may also access free testing in the community.

The Federal government is also providing up to four free at-?home COVID-?19 tests to every 

home in the U.S. beginning January 19, 2022. The tests are completely free, there are no shipping costs and you don’t need to enter a credit card number to complete your order.
